Create Form

From this section you can manage your form software. If you would like to add customized fields such as name, address then click on the Customized Fields link. If you would like to setup a confirmation email to be sent to your users when they sign up then click on Confirmation Email link below.

Link Description
Form Name This is the displayed name of the form.
Allow admin to approve users before adding them to main database Checking this box will require approval of new users when they sign up with the form.
Give users the option to review information before the form is submitted

This option means users will be taken to a page displaying their information before their signup is complete.

Add Captcha protection Checking this box will add a Captcha box at the bottom of the form, requiring users to input the characters in order to signup.
Forward users to this page when they sign up This is the URL users are forwarded to after signing up with the form.

Once you create a form you can edit the individual fields available to users.

When you add or edit a field you will have the following options.

Link Description
Field Name This is the display name of the field.
Required If this is checked the user must enter something in this field for the form to be submitted.
Allow user to modify

You can set whether users can modify the field here.

When users do a search, show them this field in results This option allows you to determine whether this option is displayed while searching.
Type This tells you what type of input the field takes. Some fields require you to add extra information, such as choices in a drop down menu.
Display to user This option allows you to use some fields for administrative tracking without users seeing them.
