| Omnistar Form Builder is a form
manager that you can use to gather email addresses
and other information from your site visitors. Setting
up the software for the first time is a simple process
that can be completed in a manner of minutes.

STEP 1: Create a new Form
First select create form. Name the form and determine
whether the administrator should have to approve new
users. You can allow users to preview their submission
before it is uploaded, and you can include Captcha
protection for the form. Finally, if you want to forward
users to a thank you page, add the url.

STEP 2: Customize the Form Fields
Click Customize Fields link. You can add new Form Builder
and rearrange Form Builder from this page. Select Add New
Form to customize your Form Builder. Add a Form Name and
select what form you would like to add it to. Select
whether the form should be required, Select whether
the user can modify the form and whether they can
search them. Add a Form Name and select what kind
of field you would like to add. Select whether the
field will be visible to users.

STEP 3: Generate the form
You can generate a form on a seperate page, or you
can generate html code for the form and put it into
your own page and customize its colors yourself. If
you select a standard form, you can generate a form
page within the software. You can edit the colors,
html and images on the page, and add text after it.
Click update and the form will be created.

STEP 4. Import
Simply return to the manage users page and edit the
list, then scroll to the bottom of the page and upload
the file you just edited. You can also upload files
that use other delimiters besides comas, simply enter
the character into the delimiter box if you are using
one of these formats.
